2020-2021 Draft Choice
SUMMER Policies: Upon registration in a FULL WEEK Summer Program, if paying by Cheque, 50% of the fee
for each session must be paid immediately, and a cheque for the balance, postdated June 15th, 2020 is required. If registering after June 15,
2020, and paying by cheque, then the full amount must paid to guarantee a spot in the desired program. If paying by credit card for a
summer program then the full amount will be charged to your card. Up until June 15, 2020, there are refunds for cancellations but are
subject to a $75 administrative fee. After June 15, 2020 there are no refunds, only partial credits towards the following summers programs
at the discretion of the office. If your child misses a day or half day during the summer programs there are no discounts or refunds. If a
player misses a session we will do our best to add them to another session in the same summer for the amount of missed time provided the
program is not sold out. Discounts are available for families enrolling more than one child from the same family (5%) in a full day
program/session. For Individuals who register for 2 half programs, you will receive a "discount" FULL week rate There are NO
discounts for half day sessions! There is a $75 charge for all "NSF" cheques. All monies must be paid in full before the player
is allowed to participate in a DCH program for insurance purposes. All refunds for cancellations are subject to a $75 administrative fee.
Absences for days will not mean refunded money or credit
Agreement & Waiver: Waiver: I agree that I shall provide health insurance or other applicable insurance to cover any personal injury and
property damage sustained by the student while participating in the activities of or while on the premises of Draft Choice Hockey Programs
Inc. and that in consideration of the services provided in connection with this ice-skating and hockey program. I / we understand and
appreciate that participation or observation constitutes a risk of serious injury, including permanent paralysis or death. I voluntarily and
knowingly recognize, accept and assume this risk and therefore release Draft Choice Hockey Programs Inc., its affiliates, theirs sponsors,
event organizers and officials from any liability. In the event that my child is injured during the absence of parents or legal guardian, I give
my permission for the person in charge to seek medical attention.

Programs -You should receive an email confirmation from the DCH office within 4 to 5 days letting
you know that your application was received. If you do not hear from the office within a week please
contact our office via email or phone call to confirm your registration was received. Occasionally the
registrations do not make it to our inbox. If you have not heard from us, not received a confirmation email,
nor received an official receipt, then chances are we never received your registration and your child did not
get into our camp nor did we receive your personal information to contact you regarding the program you

If you chose Cheque as your method of payment, then your registration will go into our pending
file until a payment is received. Your child will not be entered into the selected
program until we receive payment. If you chose a credit card as payment then the card will be processed within
2 to 3 weeks and a confirmation will be forwarded to you. Please keep this confirmation
for your records as proof of registration and official receipt. Cheques should be made payable to:
